What is in the resident pack
21 documents for one resident, produced from the answers already recorded. Every one prints under your Registered Provider’s letterhead, with your own business named inside it.
The documents
Numbered as the paper pack numbers them, so a support worker moving from paper to this app is looking for the same form under the same name. Nothing here is re-keyed: each one is built from what has already been entered for that resident.
HM-1
Initial Needs Assessment
The interview a support worker fills in with the resident when they arrive. Its answers decide which risks the risk assessment has to score.
HM-2
Excluded License Agreement
The agreement the resident signs. The room, the weekly licence charge and the service charge come from their record, so the figures on it are the figures the account holds.
HM-4
Housing Benefit Authorisation Form
Consent for the council to discuss the housing benefit claim. It names the Registered Provider and the managing agent in the same sentence.
HM-5
Missing Person Form
Description, photograph and contacts, prepared before anybody needs them rather than on the night.
HM-6
Nil Income Form
For a resident declaring no income.
HM-7
Your Weekly Service Charge
What the weekly service charge covers, what the arrears ceiling is, and what happens on reaching it.
HM-11
Your Support Plan
What the resident wants to achieve, the barriers in the way, and the steps agreed with them.
HM-12
Resident Review Form
The periodic review. The date the next one falls due runs from the last review held, not from the day the plan was written.
HM-13
Support Session Record
One visit, call or meeting, with what was discussed and how long it took.
HM-14
Support / Contact Log
Every session and every contact attempt on one sheet, in date order.
HM-9
Your Risk Assessment
The risks scored with the resident, and what is being done about each.
HM-10
Risk Assessment for Staff Eyes Only
The same assessment’s staff-only notes, kept on their own sheet so the resident’s copy can be handed over without them.
HM-20
Resident Exit Form
Completed when somebody moves on: where they went, what was returned, what is outstanding.
HM-15
Incident Form
Belongs to the property rather than to a resident, because an incident may involve nobody who lives there and has to outlast everyone who does.
HM-3/19/21/22
Checklist
Four lists, each ticked and dated: fire safety induction on the day they move in, knowing your rights, the room inventory, and the identity documents seen.
HM-16
Missed Meeting Letter
Sent after a missed appointment.
HM-17
Warning Letter
The first written warning about non-engagement.
HM-18
Final Warning for Non-Engagement
The last one before a licence is ended.
Termination Letter
Termination Letter
Ends the licence, with the notice period and the date it expires.
Note to Tenant
Note to Tenant
A short note to the resident, on the same letterhead as everything else.
Payment Details
How to pay
How to pay, from the bank details recorded for the Registered Provider. Produced only once those details exist — there is nothing to print but a gap otherwise.
The four checklists
One entry above covers four lists, because in the app they are the same object four times over — a list of statements, each ticked, dated and signed.
HM-3
Fire Safety Induction
HM-19
Knowing Your Rights
HM-22
ID Check List
HM-21
List of Inventory
Whose name is on them
The Registered Provider prints at the top of every page, as the letterhead. Your own business prints inside, where a council looks for whoever manages the house. You enter both once; every document from then on carries them.
Change the provider on a property and the pack as it stood is emailed to you before anything is rewritten, so the version a council was given is still a version you hold.

A complete pack for a made-up resident, with every form filled in, is on the front page. It is the real output of this app, not a picture of it.
